Question for the disabled

What types of accessible items are you sick of seeing here in our real world? Things that are all over the place but you don't find to be particularly useful, or perhaps are even more detrimental to the disabled than others?

The reason I ask is that I'm writing a story where accessibility just isn't a big concern, and I want to explore what types of accessibility people would find annoying or hinderances if they were so common. Some of them I have are ramps placed in spots that don't make sense, wide, overly rounded doorhandles that are impossible to grip without feeling like you're trying to palm a basketball, loud alarms blaring at random times rather than at times that actually make sense.

But I want to hear from people with disabilities. What accessibility items do you encounter all the time that you find are completely useless or more of a hassle for you than anything else?
