How to get good at a language you already know

I am a native English speaker and have a bachelor's degree in Japanese, but I have an odd problem. I've spent years of my life studying Japanese, I've lived in Japan for years, but my Japanese skills aren't great. I have a good understanding of the language, I can understand as much as 70% of a Japanese television drama, but I have a lot of difficulty speaking, and I mostly think in English. I have similar issues with other languages I've studied, even if I lived in the country. <br><br>I really want to get past this, but I can't find any consistent ways of improving. Has anyone else run into this? What did you do to get over this hurdle?
