What should a 22F ask her doctor about getting sterilized?

I have a appointment tonight and I’m going to be talking to them about my birth control. I’m about to move to the implant and I don’t want any of my family to know. I also want to get my tubes tied and never have kids. I want to know how to talk to my doctor about it. I evidently have to go through counseling first, but I want to know what questions I should ask specifically.
