Which Japanese voice actors have received lifetime bans from their voice acting agency, and why?

The most prominent one I know of is Ayumi Fujimura, though it's due to the reason of dying of a drug overdose in 2013.<br><br>I can't think of any others, but please let me know if you know of any other instances of this happening.<br><br>Also, I'm sorry if there's already a thread on this topic on r/anime.
