Fatigue report

I’m in software testing and I would regularly write a fatigue report to report to my manager when I’m at a point where I’m not able to concentrate in the work I’m doing. <br>I think in today’s generation people would be uncomfortable in raising their voice on mental health problems and I think this app would help people to come out and be open about it.<br>This app helps people to write a fatigue report and the report could be mailed to the reportee and could be tracked on the app.<br>The app could also provide suggestions on how people could reduce and tackle fatigue.<br><br>Edit: <br>I just want to share an example of what a fatigue report would look like. It could be a reportee or anyone or anything)<br>**Effect of Fatigue :**<br>(How does it effect your work/your personal life/your relationships)<br>**Measure to be taken :**<br>(What do you think would be a better solution and how would you tackle it)<br><br>Example of a fatigue report could look something like this. <br><br>Cause for Fatigue : <br>My reportee isn’t being very understanding to me and is continuously asking me for a report. I feel very uncomfortable in sending a report on my mental health and reportee isn’t being considerate.<br><br>Effect of Fatigue: <br>I’m being unstable and I’m continuously getting migraines and feels stressed. My personal life is also in disarray. I’m not able to sleep in the night properly and I’m continuously waking up. <br><br>Measurement to be taken: <br>I think my reportee should communicate in a better way and be more considerate and reportee isn’t being understanding and doesn’t want to communicate properly. I think the work or the task I’m doing now should be assigned to someone else and I should be reported to someone else. <br><br>Example: <br><br><br>I hope it’s clearer now.
