Do people who are in hospitals and hospitals themselves rely on a paywall to pay for some equipment?

Let's say hospitals need certain equipment, so they either ask certain people to pay for it (a paywall) or they ask suscription based (e.g. pay this amount per year and we'll give you this equipment so you can pay for more etc etc).
