Why do I so rarely see stories of people accidentally having a baby while happily using good birth control?

I get it, accidents* happen. And boy do I love reading all the lovely stories on this sub of happy surprises.

But I can't totally shake the feeling that birth control is designed to work so well that the vast majority of 'whoops' stories on this board probably involve a level of choice.

Which leads me to my question: where are all the stories of people who were using good birth control that actually failed, with no lapses in use or planning?

I've worked in reproductive health for years and I've never even heard of someone getting pregnant off of a Mirena or Nexaplinon regimen. Similarly, I know several people on Tri-cyclen Lo and other forms of the pill who have used it continuously for decades with no failures.

Has anyone here had birth control fail them without a fault? Do you know someone who has? I want to hear about it.
