What Am I Do Wrong Here

I'm in my late 20s and I make about 70k. I feel like I'm going insane, because I should be able to afford basic things and I just cannot. I have no student loan debt, no car debt, and no credit card debt. I don't do drugs, and I don't drink much. I earn extra money through voice over work, dog walking, and freelance writing. I see therapists regularly. I do have some medical issues, but I don't think that's the root issue here.

I like to think I have common sense. I don't really buy fancy clothes. I don't travel. I try to be smart about money. I make probably around 90k a year. I do have some medical expenses, but I don't think that's that root issue here.

I've been shopping around and just don't get HOW people afford things. Here are examples:

1. Gym membership. I try to keep my weight in check. I want to go to the gym. I don't want to pay that kind of money every month. I don't think I should have to pay to go to the gym. It should be like paying a library fee. With this in mind, I went to 10 gyms. The cheapest ones were expensive still. The best deal was 80 a month and that was the cheapest I could find.

2. Health insurance. I have a job and my company does pay me health insurance. I am moving to a job where I have to pay my own health insurance. I got quotes from several places. The cheapest one is 500 a month.

3. Cell phone. I went to several cell phone providers and that is 80 a month in some cases.

4. Streaming. I know you can just steal movies and TV shows, but I don't want to do that. I want to pay for them, but there's Netflix, Hulu, Disney+, Apple TV, and various other ones. If I calculate all of these together that's like 100 a month.

These are just examples. I think part of the issue is that you go with the cheaper option, but then you have to pay extra for things. For example, the cheapest health insurance option might be 400 a month, but you have to pay extra for any doctor visits and prescriptions and copays. The 500 a month included all of that. These add up, and I don't understand how you're supposed to afford anything else.
