NANA stated he is currently attending therapy and is now an alcoholic. Should this make him a unreliable narrator?

I was wondering if his condition makes him an unreliable narrator? My opinion was always that you take what big Tony says with a grain of salt but now with him stating that he is a drunk I feel it makes his statements less believable and sound like things he would say under the influence.
