How up to date were medical practitioners of the time supposed to be?

I've heard that for most of human history, even up to the early 20th century, it wasn't uncommon for medical practitioners to be out of date just a few decades. <br><br>Edit: this is mostly from anecdotes of medical students who were mocked for passing on "novel" theories of the time.
