I am very afraid to ask this, but how do you guys use your condoms?

This might be TMI, but I use a condom, put a couple of drops of lube on the tip and my girlfriend rubs lube all over me before we do the deed. The problem is, my girlfriend is very active, and sometimes she gets a bit rough, I can’t help but wonder if it will slip or what will happen to the condom if she really goes hard at it? I’ve never had an issue, but I just want to know.
