I think I have lice and I don’t know what to do

So I (27F) have lice and I don’t know how to get rid of it, or what to do, or who to tell. I have been dealing with this for a month and it’s driving me crazy. I don’t want to go to the doctor because I’m afraid of the outcome, but I don’t know what to do at this point. I have tried the over the counter medication that I got at the pharmacy, but it didn’t work. I am afraid that I have to tell my husband, or my family and friends, but I’m not sure if I’m ready to face that. I am also afraid that I have to shave my head, and I don’t know if I can do that. I have also tried to try over the counter shampoo, and that didn’t work either. I am getting desperate and I don’t know what to do. I am hoping that someone on this subchambers can help me.
