What happens if you’re being discharged from the hospital and you are homeless?

I heard a story of a nurse giving a homeless patient a coat to stay warm after he was being discharged back to the streets. I’m assuming the hospital can’t force a patient to stay but I could see them not wanting someone to leave without enough supplies to keep themselves warm and healthy. <br><br>What exactly happens to people without homes after they leave the hospital? Are they given any supplies?
